What is Fundus Fluorescein Angiography ?

Fundus fluorescein angiography is a special retinal imaging test. A fluorescent dye is injected into a vein in the arm and photographs are taken as the dye circulates through the blood vessels of the retina. The dye moves through healthy and diseased vessels in different ways so that the images formed clearly show regions of leakage, blockage or abnormal new blood vessel growth.

This detailed vascular mapping is especially helpful in conditions where the problem is with the retinal blood supply itself, providing information structural imaging tests such as OCT cannot provide alone.

Conditions Assessed with FFA

When FFA May Be Recommended

This test is generally recommended when the patients present with:

  • Diabetes with signs of changes in the blood vessels of the retina on examination
  • Sudden or progressive loss of vision with suspected vascular origin
  • Distorted vision in the central visual field – suggesting a macular problem
  • Further vascular characterization of retinal OCT features
  • Known retinal vein or artery occlusion requiring detailed mapping
  • Abnormal new blood vessel growth in the retina (possible).

Conducting the FFA Test

The pupils are dilated and a fluorescent dye is injected into a vein. A rapid series of retinal photographs is taken as the dye moves through the retinal blood vessels over the next few minutes. The images are carefully reviewed to identify leakage, blockage or abnormal vessels, which then helps to guide further treatment decisions.

What Happens After FFA Testing

Treatment may include: Depending on the results,

  • Laser photocoagulation aimed at localized sites of leakage or abnormal vessels
  • Intravitreal injections for abnormal vessel growth or macular edema
  • More intensive follow-up of the evolution of diabetic retinopathy
  • Referral for vitrectomy with significant vascular complications
  • Coordination with a physician to improve systemic control of diabetes or blood pressure, as applicable
